Layer 01 · Outcome domains
Four broad domains describe the directions a transition plan ultimately serves. They are reference points — not eligibility categories, not targets to be scored. Each learner's mix of priorities across these domains is unique.
Pathways into work, vocational identity, and productive contribution as an adult — including paid employment, self-employment, and other meaningful productive roles.
Living arrangements, daily-life management, self-care, finance, health, and home management to support autonomy across the lifespan.
Active membership in civic, recreational, and social communities — using transport, services, and public spaces alongside peers.
The capacity to direct one's own life — choices, goals, and a sense of meaning, well-being, and personal agency that frames every other outcome.
Domains are intentionally broad. Concrete signals of progress toward them are described by the readiness indicators and made developmentally specific by the stages.
